Application Form for Exemption from Customs Duty,

Special Consumption Tax, and Value Added Tax

In accordance with Paragraph 2 of Article 121-3 of the Special Tax Treatment Control Law,

this application is summited by the undersigned.

A copy of a document certifying that the project concerned is eligible for an exemption or reduction of

the income tax, etc., under Paragraph 1 of Article 121-2 of the Special Tax Treatment Control Law

A copy of a document cerifying that the capital goods concerned fall under the description stipulated

in each of the items of Paragraph 1 of Article 121-3 of the Special Tax Treatment Control Law

A copy of a specification of the imported goods, including capital goods certified in accordance

with Paragraph 2 of Article 38 of the Decree of Foreign Investment Promotion Act.
